What is an Auditory Brainstem Response (ABR) Test?
The ABR test is an advanced, objective evaluation that tells us exactly how the inner ear (the cochlea) and the brain pathways for hearing are working. Unlike standard hearing tests, you do not need to respond or press a button. In fact, the test is best performed while the patient is fast asleep!
We gently place small, comfortable sensor stickers (electrodes) on the forehead and behind the ears to measure brainwave activity in response to sounds. It is the gold standard for diagnosing hearing loss in infants and for detecting specific nerve-related hearing issues in adults.
When is an ABR Test Recommended?
Your pediatrician, ENT, or audiologist may recommend an ABR evaluation if:
- Your newborn received a “Refer” (fail) on their initial OAE screening.
- Your infant or toddler is showing signs of speech and language delays.
- An adult has asymmetrical hearing loss (hearing is noticeably worse in one ear).
- A patient is unable to complete a standard behavioral hearing test.
- There is a need to rule out auditory neuropathy or other nerve-related conditions.

How the ABR Test Works
The process is 100% safe, non-invasive, and completely painless for both infants and adults.
Step 1: Preparation.
For babies, we recommend keeping them slightly tired before the appointment so they easily fall asleep once the setup begins.
Step 2: Sensor Placement.
We gently clean a few spots on the forehead and behind the ears, then apply three small sticker sensors (electrodes).
Step 3: The Sound Test.
Soft earphones are placed in the ears to play a series of clicks and tones. The sensors quietly measure how the hearing nerve responds to these sounds.
Step 4: The Results.
Once the test is complete, our audiologists will interpret the brainwave graphs. We will clearly explain your child’s hearing levels and guide you through the next steps or assistive technology options if needed.

The actual testing takes about 30 to 45 minutes, but we usually block out 1.5 to 2 hours for the appointment. This allows plenty of time for the baby to feed, settle, and fall into a deep sleep before we begin.
They are actually the exact same test! ABR stands for Auditory Brainstem Response, while BERA stands for Brainstem Evoked Response Audiometry. Different doctors use different acronyms, but the procedure and results are identical.
